How your PhilHealth premium is worked out

PhilHealth charges a flat 5% premium rate on your monthly basic salary, split evenly — 2.5% comes out of your pay, and your employer matches it with another 2.5%. That 5% only applies within a basis floor of ₱10,000 and a ceiling of ₱100,000: anyone earning below ₱10,000 is charged as if they earn ₱10,000, and anyone earning above ₱100,000 is charged as if they earn exactly ₱100,000.

That ceiling means the most anyone pays as an employee is ₱2,500 a month, no matter how high their salary climbs past ₱100,000.

Is the premium based on gross or basic salary?
PhilHealth premiums are computed on monthly basic salary, not on allowances, overtime, or other variable pay. Check your payslip — the premium basis should match your basic pay, not your gross pay.
Do self-employed and voluntary members pay the same rate?
Self-employed, voluntary, and OFW members pay the same 5% rate but declare their own monthly income within the same floor and ceiling, then remit the full 5% themselves since there's no employer to split it with.
